Chronic Condition Follow-Up
A Chronic Condition Follow-Up is a focused visit designed to help you stay on top of ongoing health issues such as high blood pressure, diabetes, high cholesterol, arthritis, autoimmune conditions, or mood-related concerns. Rather than waiting for problems to escalate, we use this time to proactively assess how you are doing and fine-tune your care.
Before your appointment, you are encouraged to gather home readings-blood pressure, blood sugar, weight, symptom journals, or flare trackers-so we can review them together. We look at trends, not just single numbers, and connect what you are feeling day-to-day with what we see in your data and recent labs.
During the visit, we will:
- Reassess your symptoms, function, and quality of life.
- Review and adjust medications when needed, including gradual reduction if it is safe and aligns with your goals.
- Discuss lifestyle factors-nutrition, movement, sleep, stress-that directly influence your condition.
- Update your integrative care plan, often with specific anti-inflammatory strategies tailored to your diagnosis.
These follow-ups may be conducted via telehealth or, for eligible patients, as part of an in-home visit. Frequency is individualized: some adults benefit from monthly check-ins as they work on new habits or medication changes; others may be stable with visits a few times per year.
Our goal is to help you feel informed and in control of your chronic conditions, rather than feeling like they control you. By consistently reassessing your plan and staying in conversation about what matters most to you, we work together toward fewer flares, better energy, and safer long-term care.
