Medication Review

A Medication Review visit is a focused appointment where we sit down together-virtually or in your home-to look closely at every prescription, over-the-counter medicine, supplement, and vitamin you are taking. As an adult/gerontology nurse practitioner with years of experience in geriatric primary care, I am especially attentive to polypharmacy and the way multiple drugs can interact, duplicate each other, or quietly cause side effects.

Before your visit, you gather all bottles, lists, and recent prescriptions. During the appointment, we go through each item one by one: why it was started, whether it is still helping, possible interactions, and any symptoms you may have noticed since beginning it. We also consider your kidney and liver function, fall risk, blood pressure, mood, sleep, and digestion, since these often reveal medication-related issues.

From there, we outline a clear, individualized plan. That might include suggestions for simplifying dosing schedules, questions to bring to your specialists, options to switch to safer alternatives, or, when appropriate, a gradual deprescribing schedule to reduce or stop certain medications with careful monitoring. We also talk about lifestyle and anti-inflammatory strategies that may help prevent new medications from being added over time.

You receive a reconciled medication list and written recommendations you can share with your pharmacy and other clinicians. The aim is not to take you off medicines you truly need, but to help ensure every item on your list has a purpose, a benefit, and the lowest reasonable risk-so you feel safer, clearer, and more in control of your regimen.
